You do NOT need to write any addresses.
Begin your letter as follows:
Dear Sir or Madam,
Student answer (Band 6.5)
Dear Madam,
I am writing you to ask for your help according to the noise from my neighbour at nights.
The guy who is living in an apartment, that is very close to mine, since last week has started regularly playing in a computer too late and too loud.
I am supposed to rest at that time cause I have to go to my work in the morning. However, I am not able to sleep enough almost every day during these two weeks.
I have tried to talk with him and to complain about this problem, but he did not change his behavior.
Therefore, if you are in touch with the guy, from the apartment number 221, please remind him our rules about quite times in this house.
I suppose the note from you may have a better impact than mine.
Do not hesitate to ask me some questions, if you have them.
Thank you in advance.
Best regards,
Ann

Task Achievement / Task Response: Band 7.0
The letter addresses all three required points: it says the noise began “since last week,” explains that you cannot sleep adequately before work, and asks the agency to remind the neighbour of the quiet-time rules. The request is clear and appropriately directed to the agency, and mentioning that you have already spoken to the neighbour helps explain why you are seeking further assistance. To make the response more fully developed, clarify the timeline: “since last week” conflicts with “during these two weeks.” You could also specify what action you want if a reminder does not work, such as contacting the neighbour formally or following the agency’s noise procedure. These additions would make the requested outcome more precise.
Coherence and Cohesion: Band 7.0
The letter follows a clear progression from introducing the complaint, to describing its effect, to explaining your previous attempt to resolve it and requesting agency action. Paragraph breaks help separate these stages, while “However” and “Therefore” make the relationship between ideas easy to follow. The closing is also suitable for a formal letter. Some links could be more precise: “at that time” has an unclear reference, and the shift between “since last week” and “during these two weeks” interrupts the otherwise clear timeline. Consider grouping the start date and frequency together, then describing the impact and your requested action in separate paragraphs. This would make the organisation even more controlled and the progression smoother.
Lexical Resource: Band 5.5
You use enough vocabulary to communicate the complaint and its consequences, including “regularly,” “too loud,” “complain,” and “better impact.” However, several choices are unnatural or imprecise for a formal agency letter, which limits the range and control. “According to the noise” does not express the intended meaning; “about the noise” or “regarding the noise” would be more natural. “Playing in a computer” is also an awkward description of gaming, and “quite times” contains a spelling error and should be “quiet hours.” The phrase “the guy” is informal in this context; “the neighbour” is more appropriate. More precise collocations and a consistently formal register would strengthen the letter.
Grammatical Range and Accuracy: Band 6.0
The response includes a mix of simple sentences and more developed structures, such as “I have tried to talk with him … but he did not change his behavior” and the conditional request beginning “if you are in touch with the guy.” Meaning remains clear, and the sentences are generally manageable. Some grammatical errors remain, however. In “I am writing you,” the verb pattern needs “I am writing to you”; “an apartment, that is very close to mine” uses an unnecessary comma before the defining clause; and “I am supposed to rest at that time cause I have to go to my work” has an awkward clause construction and article use. The timeline also needs consistent tense and duration. More accurate sentence patterns and careful proofreading would improve control.
