Write a letter to the manager of the hotel. In your letter

• say where you think you left the papers
• explain why they are so important
• tell the manager what you want him/her to do

Begin your letter as follows:

Dear Sir or Madam,

Student answer (Band 7.0)

Dear Manager,

I am writing to inform you of my missing yellow card vaccination document which I left behind during my stay in your hotel, on Tuesday last week in Room 112.

As a result of rushing to meet my flight I left my yellow card vaccination card with few other document in my room. I must have forgotten these document in the bed side drawer. Its a yellow booklet with my name and document number on the cover page. Also, I have my photograph In the first page, which you can use to perform security verification.

Therefore, I would appreciate if you personally pay attention to this request because the document is one of the crucial credentials that qualifies my official assignment. Moreover, I need this yellow card on my way back, as it is a required document during immigration check.

Furthermore, please kindly check in my room bed-side drawer, that's exactly where I left it. So, if found, kindly contact my office, I have sent my company details during my check-in procedures. where possible, take pictures of the document found and ensure whoever received the document from you, should register his or her signatures as a confirmation that the document has been received.
